The Core Concept of the Epital Care Model (ECM)

ECM stands for “The Epital Care Model”—an operational, data-driven, person-centred healthcare service model designed to ensure genuine citizen-centred care throughout all stages of chronic disease.
The core idea behind the ECM is that every person living with a chronic condition—regardless of its severity—should enjoy the greatest possible freedom, security, and independence despite the challenges of illness.
In brief, the model builds on close collaboration between the citizen and healthcare professionals. The citizen provides daily health data, which are analysed by a background data model that alerts clinicians to any signs of deterioration. Using telemedical tools and home-delivered medication, clinicians can then initiate timely treatment—often directly in the patient’s home—reducing the need for hospitalisation or other healthcare encounters.

The ECM model is evidence-based and developed in research collaboration with the University of Copenhagen. Recently, the first randomised controlled trial (the TEMOKAP study) was completed, scientifically demonstrating the effect of the ECM model.
